Our Purpose
To collaborative with community representatives in identifying community health priorities that improve the overall health status of Western New York by addressing access, quality, experience and cost. We serve as the catalyst to recognize, mobilize, and report on community driven health improvement initiatives as supported by performance indicators.
 
Our Goals

• Identify and prioritize opportunities that will improve overall health status of WNY
• Develop and support a community-wide health data resource capability that enables planning, implementation, engagement and reporting
• Recognize the community in implementation of health improvement initiatives
• Mobilize improvement initiatives that are community oriented
• Lead health improvement initiatives that require community leadership, collaboration and innovation
• Communicate on the health status and accomplishments of community health improvement initiatives
• Develop position papers that support community health improvement opportunities to achieve positive results
• Deliver an ongoing, sustainable, region-wide health planning capacity in Western New York that:
            ⇒Utilizes reliable and consistent information and analytics
            ⇒Improves access, quality, expense and cost of care
            ⇒Promotes and supports health improvement and transition of the care delivery and financing system to focus on:
                - Building community coalitions
                - Seeking grant funding for prioritized improvement initiatives
                - Recognizing community initiatives
                - Communicating results of improvement initiatives and community best practices
• Positioning WNYCHPI as an active respected voice in other community driven projects, accomplished by developing and implementing the required competencies of:
           ⇒Planning capacity to frame the community assessment and agenda for prioritization, implementation, evaluation and reporting of improvement strategy
           ⇒Evidence Based Health Planning tools to provide focus, discipline and support benchmark reporting.
           ⇒Community Information Management System (owned and/or sourced) to support community base data analytics, planning and results reporting
           ⇒Robust Data Analytics to identify opportunities for improvement, support community/ provider driven planning and project management, enhance decision making and report results
           ⇒Results reporting framework to support transparency of health information including access, quality and cost for individual, providers, employers and other key stakeholders
