Overview

WNYCHPI is convening and facilitating a series of collaborative efforts comprised of leading health care and community stakeholders in Western New York working together to improve health (and health care) in our community. Participants in these initiatives are working together to develop and share quality improvement tools, strategies and best practices.

 

The objectives of these initiatives are to:

  • support the NY state and national health reform agenda
  • improve the health of our community and positively impact quality and affordability of care
  • ensure that WNY is recognized as a leader and a model for community health and health care

 

The WNYCHPI Planning Initiatives are:

 

Community Medicaid Collaborative – This initiative seeks to improve the health of a defined community through healthy living and an accessible, person-centered accountable health care system. Click here to read more.


Emergency Room Throughput – This initiative seeks to reduce Emergency Room waiting times and decrease Patient Left Before Being Seen (LBBS) rates. Click here to read more.

Hospital PQI –This initiative utilizes inpatient discharge data to create a “screening tool” to help flag potential health care quality issues. Click here to read more.
